On October 28, a live UBE decompression surgery for cervical spine was performed by Director Zhang Shujun at Wuxi Ninth People's Hospital. The surgery was perfectly completed within one hour and with less blood loss. Prof. Mayer, the Worldwide Minimally Invasive Spine Master, gave high praise to Director Zhang's excellence in surgery. Director Zhang repeatedly expressed his great thankfulness to his hospital, to his team, and to his respected mentor, Prof. Wang Wenjun. Director Zhang had been to the United States for five years to pursue his PhD and obtained a full scholarship there. In 2016, he flew to Munich to study minimally invasive spine surgery techniques at Sch?n Klinik München Harlaching as a visiting fellow. This is a world-renowned orthopedic hospital founded by Prof. Mayer in 1998. The global live broadcast was also promoted on AO Spine platform. Prof. Mi Jingyi, Vice President of Wuxi Ninth People's Hospital, specially came to the site to deliver a welcome speech for the conference. Dr. Zhuang Yin was the moderator of the conference.

Mayer教授分享了《UBE/BESS當前作用及展望》的專題演講。Mayer教授認為要深切了解UBE/BESS需要回顧一下脊柱減壓技術的發(fā)展與轉(zhuǎn)變，并分析了從70年代至今，從開放手術、微創(chuàng)手術、微創(chuàng)輔助內(nèi)鏡手術到全內(nèi)鏡手術的發(fā)展歷程。UBE/BESS手術優(yōu)點顯著，術后疼痛和創(chuàng)傷少，VAS評分、出血量、住院時間等表現(xiàn)出色，深受全球脊柱外科醫(yī)生推崇，特別是中國與韓國的脊柱同道，他們在UBE/BESS臨床上的探索貢獻突出。Mayer教授總結到UBE/BESS填補了全內(nèi)鏡手術和顯微鏡手術的空缺，未來有非常大的發(fā)展?jié)摿?，特別是運用在融合和重建技術上。

At the conference, Prof. Mayer shared a scientific lecture, titled "Current role and perspectives of Biportal Endoscopic Spine Surgery(BESS/UBE)". He said that a deeper understanding of UBE/BESS requires a retrospective of the development of spine decompression techniques. So then he showed us the technological transformation from 1970 to 2020 of lumbar (cervical) decompression techniques, which can be categorized as open surgery, micro surgery, micro-endo surgery and full endo surgery. The advantages of UBE/BESS are obvious. It is less invasive with less pain, less blood loss, lower hospital stay, etc. Its advantages make it popular among spine surgeons worldwide, especially the surgeons from China and Korea, who have contributed a lot to explore the application of UBE/BESS in clinical field. Prof. Mayer concluded that the UBE/BESS fills the gap between full-endoscopic and microscopic surgery, and has a great potential for lumbar fusion and reconstruction techniques. 

隨后，彭偉博士介紹了無錫九院脊柱外科團隊一年半以來應用UBE技術治療頸椎疾病的經(jīng)驗分享，并點評到UBE術中水流要流暢，保持持續(xù)灌注和合適水壓可降低術后頭頸部疼痛的風險，一個通道可解決兩個節(jié)段，并要注意熱損傷。崔巍博士在手術直播環(huán)節(jié)也介紹了手術病人的基本情況：患者65歲男性，臨床診斷為C4/5/6神經(jīng)根型頸椎病并頸椎間盤突出，確定治療目標為UBE鏡下行C4/5/6減壓進行椎管減壓和神經(jīng)根的松解，隨后張樹軍主任聯(lián)同韓曉飛醫(yī)生共同完成此臺精彩的手術直播。至此，中德脊柱外科手術全球直播完美落幕。

Later, Dr. Peng Wei shared the experience of Dept. of Spine Surgery of Wuxi Ninth People's Hospital in applying UBE technique for cervical diseases in the last 18 months. And their team commented that during UBE surgery, water flow should be easy and smooth; maintaining continuous irrigation and appropriate water pressure could decrease head and neck pain risk after surgery; one working portal for two segments could be realized, and thermal damage should be carefully considered. During the live surgery session, Dr. Cui Wei first introduced the patient's information: he is 65 years old, and is diagnosed with C4/5,5/6 cervical spondylotic radiculopathy and cervical disc herniation. Their surgical strategy is to do Unilateral Biportal Endoscopy for C4/5, 5/6 spinal canal decompression and nerve root release. Then this live surgery was perfectly completed by Director Zhang and Dr. Han Xiaofei in the OR. At this point, Sino-German Spine Surgery Global Live Broadcast came to a perfect conclusion.

Director Zhang always has a dream: his team, his department could be seen by the people in Wuxi City, and people all over China and all over the world. They desire to learn from the masters and to better serve the patients. He left a message in our Wechat group, "To be a good doctor is indeed a lifelong career." Only talents could build up a strong healthcare system, and only a strong healthcare system will nourish a strong country.  

In November, a series of healthcare talk videos specially recorded by Prof. Mayer will be released for the first time at Wuxi Ninth People's Hospital. We envision letting more Chinese patients to truly understand various spine diseases through Prof. Mayer's video and hope to increase the influence of the Dept. of Spine Surgery among the patients in Jiangsu Province.
